Patient Responsibilities

Ensure you bring my child's health card to every visit.

You are required to present your child’s health card at every visit in order to be seen.

It is very important that you ensure your child’s health card is valid and not expired.  Children’s health cards expire when they turn 2 years old and again when they turn 9 years old.

Please visit https://www.ontario.ca/page/serviceontario to renew online, or visit your nearest Service Ontario.

Ensure you wear a mask while in the office if you have infectious symptoms. 

We no longer require masks to be worn in the office, however, we ask that you wear one if you have infectious symptoms.

Ensure you call to cancel or reschedule if you cannot make it to your appointment.

In order to ensure the office is running smoothly, and to continue to accommodate as many patients as possible, there will be a charge for missed appointments.  Please call the office to cancel or reschedule to avoid being charged.

Ensure you book appointments in advance whenever possible.

Well baby visits are booked at 1 month, 2 months, 4 months, 6 months, 9 months, 12 months, 15 months, and 18 months.  After this, well child visits start at age 2 years, and occur once per year.  Any patient taking regular medication (ex. For ADHD, mental health, etc.), is required to be seen in-person at least every 6 months.  No medication refills will be sent to the pharmacy without being seen.

Ensure you bring the proper documentation.

If you received a letter from the Health Unit for vaccines, please bring this with you to your appointment so we know which vaccines are required.  Please bring any parent or teacher questionnaires that Dr. Danby has asked to be filled out prior to your appointment.

Ensure you report all vaccinations received to the Middlesex London Health Unit.

It is the parent or guardian’s responsibility to report all vaccines.  Our office does not do this, but we will fill out your child’s yellow card at each visit if you have it with you.  We do have records of vaccines given in our office, but not of any vaccines received elsewhere.  To report vaccines only, go to https://www.healthunit.com/immunization-records.

Ensure you obey the office safety and harassment policy.

MD Pediatrics is committed to providing a safe, secure and respectful environment for all patients and staff.  We have a zero tolerance policy for words or actions that make others feel threatened or demeaned. This will not be tolerated, whether in person, by phone, or any other means of communication.  These include, but are not limited to:

·         Abusive language and yelling

·         Violence (physical attacks or threats of harm)

·         Intimidation

·         Throwing, damaging property, or breaking things

·         Unwelcomed physical contact

·         Threats or threatening behaviour

·         Refusing to leave the property

Immediate action will be taken when incidents described above occur, and you and your child may be dismissed from this practice.
